Floating Support Service - Enabling independence in the Community

About this service


The Floating Support Service offers vulnerable people, the opportunity to improve their quality of life, by enabling them to live more independently within the community.

The Floating Support Service helps people to remain living independently within their own homes.



Referral

People can be referred to us through their G.P., Psychiatrist, Community Mental Health Teams or any Care Services.



What the service offers

The service is tailored to individuals needs based on the following elements:
  • Providing visiting support

  • Assist with compliancy of terms and conditions of tenancy

  • Enable, maintain safety of dwelling

  • Enable, maintain security of dwelling

  • Liason with relevant agencies and mental health professionals

  • Provide relevant advice and guidance to assist the claimant


Floating Support Project

We have a small team of 2 Floating Support outreach workers. The above team compliments our day service provision and the Carer Support Service.
